Nursing Plans and Care (Hospice Nurse): Nursing Medical assessment tool for clinical notes and records. Very small a nice, it can fit into the pocket of Clinical code.

I was expecting more clinical rationale and guidance but this was a workbook with blank tools for assessment and care planning.

To be fair, I have not read the book in it's entirety. The cover image turned me off. Rarely do we use needles in hospice care. It's part of the whole ethos.
