Mine had 5 rings. One with letters, the other 4 all numbers.

how does one reset a combination on the lock?

Put the combination in and one of the ends will be able to slide off so you can tale the individual pieces off and put them in the order you want.

Stopped working... locked in the unlocked position. i can't get the arm to swing. i haven't forgotten my code. sigh. any suggestions?

When my lock got stuck I used wd40 and it loosened everything up. It got a little too loose so use the oil sparingly.
