Why don't these fit into patch panels that accept other keystones?

I did not use these in patch panel, the were used in Keystone wall plates, sorry a can’t be of further assistance.

Will these work with a 70v speaker system?

yes, only limitation is wire size, great product, ( I used with 100v system)

Do these not fit standard patch panels. I tried and they seem too big

they are for wall plates, not panels
